PCAV Membercard Replacements

If you need a Replacement Grading Card please use this form

If you have trouble downloading the form then an email with Rider Name, Club and where it is to be sent will suffice - email to membership@ponyclubvic.org.au

Alternatively, if you need it quickly please phone the office (during office hours 9-5 Monday - Friday) on 8685 8925 to order one.

We need the full name of the rider and where the card needs  to be sent (bear in mind that it needs to be signed by 2 club officials).

There is a $5 charge for Replacement cards.
